New Patient Registration

If you would like to become a patient at Wycliffe Surgery then please print off and complete the following documents and return to the reception desk at Wycliffe Surgery:

You will need to present TWO FORMS OF IDENTIFICATION along with your COMPLETED FORMS to the surgery before your registration will be approved:

  • A passport or a full driving licence
  • A recent utility bill with your name and address on

*Please note if you are registering a newborn child then you must provide us with the NHS number (this will be given to you when you register the birth).
