Register with our practice

Anyone in England can register with a GP surgery. It’s free to register.

You do not need proof of address or immigration status, ID or an NHS number.

We are a motivated, caring team who endeavour to provide you with excellent patient-centred service under our care.

Please submit an online New Patient Registration Form or you can visit the surgery to newly register.

You do not need proof of ID to register with a GP, but it might help of you have one or more of the following:

  • passport
  • birth certificate
  • HC2 certificate
  • rough sleepers’ identity badge
  • hostel or accommodation registration or mail forwarding letter

If you’re homeless, you can give a temporary address, such as a friend’s address, a day centre or the GP surgery address.

Please ensure you have 4 weeks worth of medication before you submit your registration form. This is to allow us enough time to book your new patient appointment with GP.

Named GP

In line with our contractual requirements all of our patients, as of the 30th June 2015, will be allocated a named and accountable GP.

What Does Accountable Mean?

Your named accountable GP will oversee the co-ordination of your care amongst the other health professionals.

Does this mean that my named accountable GP will take 24hr responsibility for my care?

No, the named accountable GP will not take on 24hr responsibility for your care, your named GP will purely oversee the care that you receive.

Does this mean that I have the right to see my named GP every time I book an appointment?

No, a patient can express who they would like to see, however, this may not always be possible due to appointment availability with that GP.

Can patients still see any GP they wish?

Yes, a patient can continue to see any GP they wish as they currently do.

Will I be informed in writing who my accountable named GP is?

No, your practice does not have to inform you in writing. If you would like to know who your named GP is then please contact the practice.

Accessing someone else's information

A parent with proxy access will be able to manage certain elements of the young person’s record, such as making appointments and ordering medication, but they will not be able to see the young person’s past appointments or clinical records. It is within the patient’s best interest for us to be aware that there may be some risk from the parent being able to see the prescription record. It is within our care to monitor and record any concerns, and we will remove access to the account if deemed appropriate and in the best interest of the patient. This will be reviewed Annually. Should you wish to request proxy access to another person’s account, please contact the surgery on 01752 229902.

On the young person’s 16th birthday

On the young person’s 16th birthday systemone will switch off all the remaining proxy access except where the young person is competent and a read code has been added and has given explicit consent for the access to continue by completing our proxy form in person.

We will obtain this consent by means of contact with the young person by letter on or just before their 16th birthday , as no other form of contact i.e. mobile or e-mail is confirmed as patients own contact.

Where a young person has already been given control over access to their online services before their 16th birthday, and their parents do not, there is no need to make any changes unless the young person wishes to do so.
