New Patients

Welcome to Wycliffe Surgery

Anyone in England can register with a GP surgery. It's free to register.

You do not need proof of address or immigration status, ID or an NHS number.

We are a motivated, caring team who endeavour to provide you with excellent patient-centred service under our care.

Please explore our website for information on appointments, the services we provide, our staff,  health advice and a wealth of links to online health resources.

Please submit an online New Patient Registration Form or you can visit the surgery to newly register.

You do not need proof of ID to register with a GP, but it might help of you have one or more of the following:

  • passport
  • birth certificate
  • HC2 certificate
  • rough sleepers' identity badge
  • hostel or accommodation registration or mail forwarding letter

If you're homeless, you can give a temporary address, such as a friend's address, a day centre or the GP surgery address.

If you need to help registering:

If you need help registering or filling in forms, call the surgery and let us know.

You could also ask for help from:

  • local organisations - for example if you're homeless you could ask a centre that supports homeless people
  • Citizens Advice
  • your local Healthwatch

Please see below for our online registration form and information regarding out of area patients.

Out of Area Patients

Whilst we are now happy to accept patients at our surgery that live outside of our catchment area, we may only be able to offer you a restricted set of services if you are able to travel, but we aren't able to offer you home visits.

We cannot register under 16 year olds who are out of area. Also, we strongly recommend that you register with a GP practice closer to your home if any of the following apply to you:

  • You are pregnant
  • You have children under 16 years of age
  • You have a severe mental illness
  • You need home visits or regular attendance at the GP practice
  • You need referral to or regular visits from local services, such as district nurses, social services or health visitors

If we feel that we cannot meet your health needs we may insist you register with a GP that can. Find more information about out of area registrations here.
