If you have been ill for 7 days or less you don’t need to see a Doctor

You can complete a Self Certification form yourself.  

If your employer doesn’t have its own form (many do) you can download this ONLINE SELF CERTIFICATION FORM. 

Please print, complete and hand it in to your employer. There is no need to see a Doctor.

Some employers insist on a Doctor’s note regardless (also known as a “Private Statement of Fitness for Work”) and this involves a charge of £20.
